When you submit the service request form, we collect the information you enter: your name, email address, telephone number, preferred contact method, whether the service is for you or someone else, the service recipient's name if you provide it, your city and ZIP code, your preferred and backup scheduling details, the service or membership you selected, the pickup location when applicable, your description of the request, any accommodations you tell us about, and the acknowledgments you check.

We do not ask for and do not want medical records, diagnoses, medication lists, Social Security numbers, or payment-card information. Please do not send those through this website.

How we use your information

• To contact you about scheduling, pricing, and invoicing
• To deliver the service you confirmed and paid for
• To keep our own business records

We do not sell your information.

Payments

Invoices are sent and paid outside this website. We do not collect or store payment-card numbers. Any payment processor we use handles that information under its own terms.

Storage and access

Requests are stored in a secure database. Only the business owner can view submitted requests. Submissions are not visible to other website visitors.

Family updates

When a membership includes updates to a family contact, we share only brief, general information, and only with the member's permission.

Your choices

You may ask us to correct or delete the information in your request by emailing peaceandeasecolumbus@gmail.com. We may keep basic records where we are required to.
